Back in 2002, the Iowa State Association of Counties website hosted an account of each county’s history, signed by its County Recorder, and illustrated with a black and white flag image. It is not clear whether these flag images were provided by the county administration or not.

It is a ~2:3 plain light (white?) flag showing a large emblem on it. This emblem consists of a sketched outline of the county map and on it, along the upper hoist side the words "Mills County", set in large black serif capitals and a large depiction of a horse drawn plough with its handler, seen from front right, slightly shifted to the hoist, and drawn in black (?) line art.
